Accommodating Special Dietary Needs in the School Nutrition Program

It is the goal of the Coffee County Child Nutrition Program to serve all students a variety of nutritious meals that are also delicious.  Breakfast, lunch and after school snacks are provided each school day and are in accordance with the USDA and State of Alabama Wellness Guidelines.  In order for a student to receive free or reduced price meals, a Free and Reduced Meal Application should be completed and federal guidelines for income and household size met.  If the federal guidelines for income and household size are not met for free or reduced price meals, a student will have a paid status. 

Paper meal applications are available at each school.  Meal applications may also be completed online with our new Point of Sale software.  Online meal applications are available at https://linqconnect.com.   If paper applications are completed, please return these applications to the school cafeteria.  Only one form should be used per household with all children listed on one form.  Applications are confidential and are processed at the Central Office. 

If your child has any food allergies or other special dietary needs, please contact the CNP department at the Central Office.  Diet prescription forms are available and should be completed by your child’s physician.  The form should be returned to the school nurse as soon as possible.  The form must also be on file in the cafeteria in order to accommodate your child’s need.  The school nurse and cafeteria staff will work together to create a meal that meets the student’s special diet.  A new diet prescription should be completed each school year and any time there is a change to your child’s diet during the current school year.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
